Assist and facilitate family meetings focused on developing practical solutions to challenges faced by aging or disabled family members. Act as advisor, mediator and advocate.
Advocacy
Act as physician/patient liaison. Represent client and client interests to various organizations and agencies. Attend meetings with or on behalf of client. Manage all related forms and documents.
Residential Advising & Relocation
Assistance with research and referrals for retirement/assisted living/supported living and/or nursing home options. Advice on adaptive devices and home retrofit solutions for independent living. Solutions customized to client needs, preferences and budget.
Shopping & Errand Services
Assist clients with scheduling appointments, shopping, etc. Assist clients with scheduling transportation or delivery. Arrange and coordinate household repairs, etc.
Check In and Reminders
Home visits, scheduled phone calls, appointment reminders, calendar management, customized services as needed and according to client budget and preference.
Power of Attorney
Hold or administer property or assets on behalf of client for the benefit of the client. Maintain medical consent authority for the benefit of the client.
